Roofing Material Change Form Edit page

I (Homeowner) am requesting permission to alter the existing roofing material on my home to the following materials, and have contracted with the listed contractor to provide and install the roof.  I have read and will comply with all of the applicable Colony Woods Homes Association's Declarations of Restrictions, particularly Section VII and Section IX, with regard to the plan approval process and required building materials.
